Alright, so my brother and myself were playing on Silent Hill 2 (PS2 version) and we were exploring Toluca Prison, when we happened to notice something. Now, I'm not sure if anyone else has noticed this, but it sounds like the creature is saying the name of a Silent Hill god.
Most people believe that the creature actually says, "Ritual". Some say it is "Are you sure". But my brother had a slightly different idea.
He believes, and he has convinced me, that the creature is infact saying, "Xuchilbara" (Otherwise known as "Xuchilpaba".
"The red god of Silent Hill and one of the beings mentioned in a painting in Silent Hill 3 created to lead people in obedience to God. A god of judgement, Xuchilbara presides over rebirth and has been known to be used in a ceremony to resurrect the dead, called the "Crimson Ceremony"." From Wikipedia.
"The Red God", also sounds like a reference to The Red Devil, or "Pyramid head"/Jimmy Stone. Pyramid Head is also in this game, so a reference to him would make some sense.
In one of the endings, James Sunderland, is seen taking a boat across Toluca lake. (The prison is located under the Historical Society, which also features a painting of Pyramid Head. The Historical Society is sited very close to the lake which James sets out on)
He crosses this lake to have his dead wife Mary ressurrected by using the "Old Gods", one of which is Xuchilbara, the god of ressurrection.
So this evidence makes it somewhat likely that the creature is in fact saying "Xuchilbara". Although the fact that it sounds similar to the word "Ritual" may have some significance.
Some claim that the creature says "Are you sure?", this could be questioning whether James really wants to get Mary back or not.

I'm convinced my theory is the most plausible. The prison creature is quite clearly garbling out the name of a failed theme park that was overshadowed in the 60's by Lakeside. That theme park called itself "Pukey World."
