Kira no Seiryu, a combination of Dragoon and the 'planned' accident between Kris and Takao.
All her outfit really is, is her Gen-X outfit with some alterations, using Dragoon's colors.
This is also the form in which Kira defeats Ming *2 at her own game.
While this form only happens once in G-rev, when Kira is with the Four Beasts Amazoness Knights in Gen-X, these forms are referred to as their 'battle outfits', for more serious battles.
I also credit the nifty thing I did with her hair to the 'Other Illustrations' Page in the Sailor Moon Materials Collection Book. Great way to cut off a picture. ^-^
