It's basically a fanart of Belbet, a character from the BL videogame "Enzai", (C) Langmaor. He's a life-sentenced prisoner in a real hell of a prison, and the long time he spent in isolation cell has driven him quite insane. So insane, in fact, that he convinced himself of being the king of some unspecified fantasy world or something.
Well, at last, now he's happy.
