It looks nice, except for the brushes on the forehead. You should've erased around the edges and worked on the blending. Maybe making the color a reddish-brown instead of straight up red.

I'd have to agree with frozen scent of love on this one. Perhaps try some different blending filters on your layers. And as always, try to work with high-res images whenever possible. The problem I have with this photo-manip is that the resolution of the photo is way lower than your blood additions.
What stands out is the absence of colour in the photo. Wonderful choice. It makes the blood stand out. It's really cool that you're upside down in the picture too, most would post the picture right side up. Thumbs up!
