A while back I made this for my flash drive (the shape is loosely inspired by it). I figure at least someone else out there may want to use it for their flash drive(s).

To make the image an icon for your flash drive, do the following*:
1. Download the ico image from http://cid-00c8b3d21f2e422b.skydrive.live.com/self.aspx/Public/autorun.ico
3. Copy the icon to the root (highest directory) of your flash drive with the name "autorun.ico".
4. Open notepad (or the text editor of your choice).
5. Enter the following:
[Autorun]
Icon=autorun.ico
6. Save the text document as "autorun.inf" in the root of your flash drive. The next time you insert your flash drive (or reboot your computer) the icon should show up.
*I am unsure about how autorun icons work in operating systems other than Windows.
