4242 was transferred from a hospital to a very VERY illegal asylum when he was alive. There he was subjected to many forms of 'treatment', including a lobotomy that eventually killed him. 42 frequently hallucinates, most of the stuff he sees are freaky monster things that cause him to freak out. One thing he sees a lot is a shapeshifting rabbit named Spot that basically is his mind trying to not go more crazy than he allready is from loneliness. Even after death, Spot shows up to comfort poor 4242 when he feels sad and lonely, even bringing flowers to cheer him up.

Yeah, I enjoyed painting the bloody padded room WAY too much.
